I fixed it as below:
Make sure you have:
certificate.crt (your domain cert)
ca_bundle.crt (intermediate certs)
private.key (your private key)
Concatenate the domain cert and the intermediate certs into one file:
cat certificate.crt ca_bundle.crt > fullchain.pem
